Weird that I can only find English and Canadian companies that do this test.
If my memory serves me correctly the US version, (ELISA) covers a few more food items than the YORK test. Also if, (and it is an IF), you can get your doctor to organise the YORK test, (in UK), then he gets it discounted to £208, in case any other readers are looking into it.
I think the most important point about the test is that you have to eat a regular diet for a few weeks beforehand. Unless you know 100% that certain foods are bad for you then you need to consume them in moderation prior to the test to ensure reliability.
Make sure you get the plastic card with IG reactive foods on it. I find it makes a huge difference when I go to eat out.

I live in the uk and had my Elisa test done by a company called yorktest. I had to pay for this myself and it was quite expensive, £242.00 to be precise. I`ve just checked for you and they have a US site. The address is www.usa.yorktest.com. All the info is on here, hope this helps!
